Some repairs may require a diagnostic visit and a return visit to complete repair once the parts have been received. If repair is found to be caused by misuse, owner is responsible for a service call and/or repair costs. Service call rate is $175.00. A minimum of one hour will be billed on all service calls.

Diagnostic call only $175

Diagnostic and repair under $650.00

Diagnostic and repair under $1000.00

No service calls will be scheduled until this form is signed and received.

We will require the following:

access to all pool/spa equipment
circuit breakers
live electrical outlet
and outdoor water source.

Any trip to the service location without all of the requirements above, will result in a trip charge of $75.00.

To schedule a service call Proper pH, LLC requires a valid credit card, which will be charged for complete balance of the invoice; including, service call fee, parts and labor.

We will notify you or a designated liaison of the completion of the requested work, we will then apply your service call charges to the card below upon completion of the work. We require 24 hour notice for the cancellation of any service, any cancellation with less than 24 hours notice will result in the full minimum charge billed to the credit card below.

You will receive a copy of the invoice marked PAID as receipt.

Cardholder permission is required to store payment card information for future use. This information is encrypted and tokenized thus making it a random number. It is held by the system used to process our credit cards. We can not accept credit card payment without this authorization. By checking this box you agree that credit card information provided is stored as described until the services and billing are complete.
*A fixed 3.5% fee to all credit card transactions will be added.

You must agree before submitting.

I understand a manufacturers’ warranty service call will only cover manufacturer defects. If a Proper pH technician finds repair is not covered under warranty due to improper installation, user error, rodent damage, acts of nature, etc., I understand and authorize that the credit card I am providing on this service call authorization form will be charged for the service call charge.
